What is your policy number?
(Most can be accessed by social security number.)
Who is you employer or are you self insured?
The questions that most be asked before you make an appointment with the dentist.
a) Will your insurance pay for your visit if you go out of network? (that means if you see a dentist that does not take your insurance, but can submit the paperwork for you. Will the insurance company still pay something? )
b) What is your yearly benefit? (that is the amount of money the insurance company will pay in your 12 month coverage)
c) Do you have a yearly or family deduct able? (that means any work done other than a teeth cleaning and exam could cost you anywhere from $25 to $100. per person or per family out of pocket right up front, before any treatment is started. )
d) Are you on a yearly or fiscal plan? (that means does your policy start or renew in January or in July? )
e) What is your coverage? (the amount the insurance company is going to pay for each treatment that is performed)
e i) teeth cleaning and exams are usually paid at 100 %
e ii) basic (root canals and fillings paid at 80 %
